SENIOR WELLNESS PROGRAM |

| Our Senior Wellness Program is designed to help your pet live a longer,
happier, healthier life. The program is designed for pets 7 years and
older, and should be performed annually. A healthy-appearing pet may be hiding symptoms of a disease or ailment. For
example, a pet can loose up to 75% of kidney function before showing signs of
illness. Testing allows us to evaluate your pet's health so we can treat
problems before they become too severe. |

| Senior Wellness Physical Exam |
| Arthritis Disease Evaluation |
| Dental Disease Evaluation |

Chemistry Tests and a Complete Blood Count |
|
These tests will allow us to survey many of the
organ systems of the body to make sure they are working normally, such as the
kidneys, liver, and pancreas. A complete blood count will allow us to screen for
infection, leukemia, and anemia. |

Thyroid Function Test |
|
This test
is useful
in diagnosing increased (hyper) or decreased (hypo) functions of the thyroid
gland. |

|
Urinalysis |
|
This assesses the health and function of the urinary system.
This is especially important in older animals to help early detection of
kidney and liver disease. |

| A complete physical exam is included. The doctor will go
over all test results with the owner, and formulate a wellness plan for the pet.
